            Hypertension is one of the major public health issues involving nearly half of the adult population in the world. Untreated or uncontrolled hypertension can even cause fatal issues like heart disease, stroke, and kidney failure (Fuchs & Whelton, 2020). Even though there are highly effective medications available, patients still come across difficulties in attaining and controlling the desired blood pressure levels (Williams et al., 2018). Implementation of these strategies will result in better hypertension management which will lead to improved patient outcomes and a reduced impact on healthcare systems. The Telemonitoring of blood pressure has become a cutting-edge approach to dealing with this issue because it permits more frequent monitoring and timely interventions in contrast to regular annual check-ups.

  2) Explain why the issue/concern is important to nurse practitioner practice and the issue’s impact on health outcomes.  Provide support for the importance of the issue from scholarly nursing sources current within the last 5 years.

            Hypertension is a huge problem in nursing practice as it is very common and it leads to deadly complications if treatment is not started on time. Nurses are pivotal in noting the patients' blood pressure, teaching the patients, and promoting sustained treatment adherence. Successful hypertension management can drastically improve patients' outcomes using reducing the risk of cardiovascular conditions, maintaining good organ function, and improving overall quality of life (Abreu et al., 2018). Overcoming the problem is the key to population health and the optimal use of healthcare resources. Nurses are equipped to incorporate and provide support for telemonitoring interventions. They may do this by offering regular feedback, changing treatment plans, and motivating the patient to take initiative in self-management.

  • P-Population and problem (What is the nursing practice concern or problem and whom does it affect?)

The target population is adults diagnosed with hypertension. The problem is managing hypertension effectively and keeping the pressure at the recommended levels, which may aggravate the risk of cardiac events and other complications.

  • I–Intervention (What evidence-based solution for the problem would you like to apply?)

The interventional plan entails telemonitoring of the blood pressure, which will be patients’ measuring their blood pressure at home with validated devices and sending the readings to their healthcare providers for review and management (Abreu et al., 2018). This permits more frequent monitoring and timely changes to treatment plans compared to yearly visit follow-ups.

  • C–Comparison (What is another solution for the problem? Note that this is typically the current practice, no intervention at all, or alternative solutions.) 

The control part is the usual standard practice that includes annual follow-up visits with healthcare providers to measure blood pressure control and also adjust treatment plans according to the need (Abreu et al., 2018). Although this strategy would be effective for treating hypertension, it is doubtful that it will be able to identify and resolve issues in a timely fashion, thus causing inappropriate management of blood pressure.

  • O–Outcome (Very specifically, how will you know that the intervention worked? How you will measure the outcome?)

The desired result is the management of blood pressure, which is assessed by the percentage of patients that have steadily achieved and maintained a blood pressure in the target range (Fuchs & Whelton, 2020). These metrics can be recorded through daily telemonitoring readings and later validated through in-clinic blood pressure measurements during follow-up consultations.
